IMA Medicare Wellness Clinic
Premier Healthcare's Internal Medicine Associates division offers FREE annual wellness clinics for Medicare Part B and Advantage beneficiaries, beginning the first year you're eligible, and each year afterwards. Clinics are held Tuesday, Wednesdays and Thursdays at the Landmark Medical Center on Landmark Avenue in Bloomington.

Blood pressure
Height/weight
Visual acuity screening
Depression screening
Functional ability & safety assessment
Update medication list and medical history
Individualized preventive checklist and plan that shows how often tests, screenings, and services are covered by Medicare Plan B.
On subsequent visits, you'll also receive a memory screening

Cost
For Medicare Plan B and Advantage patients, there is no out-of-pocket or co-pay expense. Keep in mind these visits are not meant to replace any medical care provided by primary care providers, but simply to give you an assessment of what should be followed up with.
